Any staff member at Summer Camp (this includes any adult at camp other than a visitor, including unit leaders) will be required to have the following documents on file with the camp director prior to beginning work at camp.

 

1. An adult leader application filled out completely (even if you are currently registered).  Adult applications are available online at New Adult Leader Application (English)

 

2. Class 1 & 2 Medical Form and if you are 40 years of age or older, you also need to have a completed Class 3 Medical Form.

    

3. Copy of Sexual Offender database check.

To obtain a Sexual Offender check at no charge, you should go to the Texas Department of Public Safety Criminal Records Service Website at: https://records.txdps.state.tx.us/DPS_WEB/Sor/index.aspx 

Enter the Sex Offender database.  Search using first name, last name and date of birth. Print out the results. (You may print by highlighting the result page, FILE, PRINT, check SELECTION box and PRINT.  

 

Take the BSA online youth protection http://olc.scouting.org/

When finished print the following documents.

 

4. Copy of your Test score page from online youth protection training.

After taking the test, click on SCORE TEST.  The next screen reveals your test scores.  Highlight the course Test Results page, FILE, PRINT, check SELECTION box and then print       

 

5. Copy of the Preliminary Certification Card page. 

The bottom of the Course Test results page reads “Once you have downloaded those items, click here to report your training to your local council.” CLICK HERE and you will be directed to the Course Completed page.  Highlight the Course Test Results page, FILE, PRINT, check SELECTION box and then PRINT.
